How to Qualify for Clinic Services

Live or work in Walworth County
Have no health insurance
Household income at or below 200% of the Federal Poverty Level

You must register before receiving services. To get started, register online or call the clinic at 262-379-1401 for assistance.

OAFC does NOT offer walk-in services.
Appointments are necessary for all clinic services – appointment times for patients are limited and vary.
